AGREEMENT: I understand and agree that submitting this application does not automatically register me as a Dawson County Humane Society volunteer. I understand that there are certain qualifications I must meet before I may begin volunteering, including, but not limited to, the acceptance of established volunteer policies and procedures.

RELEASE & CONFIDENTIALITY: I hereby fully and forever release and discharge the Dawson County Humane Society, its agents, employees, directors, officers, volunteers, and liability carriers, from all actions, damages, or judgments which I have now or in the future against Dawson County Humane Society, for all personal injuries to myself, unknown or known, and/or arising out of activities of myself as a Volunteer of Dawson County Humane Society. I understand I will not be considered an employee of Dawson County Humane Society while performing such service and understand that I am not to be paid any compensation whatever for the service I shall perform. I agree I will hold in strict confidence and not share or disclose all information designed or implied as confidential by Dawson County Humane Society and any information related to the identity of Dawson County Humane Society customers/donors, business plans, inventories, or financial records as long as such information is not generally known to others outside of Dawson County Humane Society. I understand this confidentiality pledge will remain in effect after any termination of my volunteer tenure, and I will deliver to Dawson County Humane Society any copies of information which could be deemed confidential immediately upon termination or resignation of my volunteer tenure. By signing below and submitting this form, I agree that the information I have provided on this form is true and accurate to the best of my knowledge. I agree that I have read the above release and confidentiality waiver and understand all of its terms. I agree to execute it voluntarily and with full knowledge of its significance.
